Gofal Cymru Care is a specialist provider and as such the vast majority of referrals for placements come to us through Local Authorities or Health Boards.
Following initial referral, a visit will be made to the individual by the care home manager to identify core needs and assess the level of intervention or support required.
A needs assessment is undertaken in discussion with the individual’s family and care team before a decision is made as to whether we can meet the needs of that person at any of our homes. This looks at the holistic needs, including medical, mobility, specialist equipment required, capacity and communication issues and the number of support hours required.
If we think we can accommodate those needs then we will formulate a Care Proposal and associated costings. The commissioning team are then invited to visit the home to discuss the individual’s needs and expected outcomes. Appropriate family members are also welcome to visit.
A transition plan is formulated which is unique to each individual. This includes a network day for information sharing to take place between relevant professionals and the home’s staff team, and trial visits by the individual to the home, including possibly an overnight stay(s).
A more thorough assessment is then made of any specific requirements which may be needed so that we can refine our care plans. Financial and medication competencies are assessed and a Deprivation of Liberty Safeguards (DOLS) application made if necessary.
There then follows the detailed preparation of service delivery, management plans and risk assessments in order that transition may be as smooth as possible.
Any extra training requirements for staff are identified and addressed and existing residents prepared for the arrival of the new person.
Emergency Admissions are considered at the discretion of the management team and their staff. This is done in line with various criteria including compatibility with individuals already residing at the home and availability of staff.
We also look at any specific needs that the individual may have that would need to be accommodated and whether we could safely meet those needs.
